Long-acting injectable cabotegravir pre-exposure prophylaxis (PrEP) is recommended by WHO as an additional option for HIV prevention in sub-Saharan Africa. Still, there is concern that its introduction could lead to an increase in integrase-inhibitor resistance undermining treatment programs that rely on dolutegravir.

Cabotegravir long-acting injectable HIV pre-exposure prophylaxis (LA-PrEP) is shown to be efficacious with a good safety profile in clinical trials. Understanding potential user preferences for LA-PrEP is important to increase uptake and persistence.

This study aimed to estimate the cost-effectiveness of long-acting injectable cabotegravir compared with tenofovir disoproxil fumarate and emtricitabine in South Africa, using methods standard to government planning, and to determine the threshold price at which long-acting injectable cabotegravir is as cost-effective as tenofovir disoproxil fumarate and emtricitabine.

Findings indicate that, while the overall number of people testing positive for HIV following CAB-LA PrEP is extremely low, approximately one in four persons diagnosed with HIV carried a virus resistant to DTG.
